Patient name: Pablo C Medical record number 022 date of birth 4/1/98 date of admission 9/1/00 Date of discharge 9/4/00 private doctor: Robinson This is a 2-year-old male with no significant past medical history who = presented to the Children's Hospital emergency room with erythema and = rash around the right eye. Mother reported a conjunctivitis for 1 to 2 = days preceding the development of the erythema and rash. Patient had a = fever of 39.2 and a CBC revealed a white count of 6.4 with 63 percent = lymphocytes. Ophthalmology service was consulted from the emergency = department and recommend a CT of the orbit to rule out orbital = cellulitis. The CT showed no orbital involvement. Blood and eye cultures = were obtained and the patient was admitted to the floor with IV = ceftriaxone and IV acyclovir. On the floor, the patient rapidly became afebrile. Blood and eye = cultures were negative. A herpes PCR eye culture sent from the floor was = positive for HSV type 1. On the floor, it was noted that the patient had = a similar rash on the right elbow which the mother reported was of = long-standing. Our impression was that the elbow rash represented eczema = herpeticum which had then spread to the skin around the eye. Over the = course of the hospital stay, there was slow improvement of the erythema = and rash. Patient remained afebrile and was acting normally according to = the parents. Patient was discharged home to complete a seven-day course = of Cefzil and a 14 day course of acyclovir. Patient was to follow-up = with the ophthalmology service in 3 to 4 days after discharge and the = parents were asked to follow-up with the private doctor. Thank you very much for this referral.
